This portal sends professional partner referrals directly to Jasmine when you press Send referral. A confirmation appears only after receipt is confirmed.

Information you share

Your referral may include family contact details, the situation, timing, budget, preferred area, notes, and your contact information. Family phone and email are optional. Share only the information needed for an introduction and that you are authorized to provide.

How your information is used

Referral details are sent over an encrypted connection and stored privately for Jasmine’s follow-up. The portal also keeps basic submission records to confirm delivery, prevent duplicates, and protect the form from misuse.
